Тронутый, помешанный. Полное выражение: bats in the belfry. Имеются в виду bats (летучие мыши), гнездящиеся на belfry (колокольня), которые начинают беспокойно метаться, когда звонари принимаются за свою работу. Это выражение означает также бред душевнобольного.

In the beginning we took Nina seriously, but soon we realised she was batty/had bats in the belfry. — Вначале мы серьёзно относились к Нине, но скоро поняли, что у неё не всё в порядке с головой.
